Epidemiología e inmunopatogenia de la hepatitis C y de la coinfección VIH/VHC
A) Hepatitis C en pacientes infectados por VIH
Respuesta inmune frente al VHC en pacientes VIH
La presencia de células T citotóxicas (CTLs) y anticuerpos neutralizantes (nAbs) frente al virus de la hepatitis C (VHC) pueden influir en el aclaramiento espontáneo y las reinfecciones por este virus, pero se desconoce el impacto de la infección por el virus de la inmunodeficiencia humana (VIH). La respuesta inmune en mucosas también es clave en la transmisión del VHC. Caracterizar esta respuesta inmune puede ser esencial para diseñar estrategias preventivas de la infección por VHC en personas con y sin VIH. Objetivos: 1) Estudiar la respuesta inmune (CTLs y nAbs) frente al VHC en distintas situaciones clínicas (aclaramiento espontáneo, reinfección, y tras la respuesta viral sostenida (RVS) con terapias basadas en interferón (IFN) o en antivirales de acción directa (DAAs). 2) Cuantificación del VHC y de ARNm de biomarcadores inmunológicos y de daño epitelial en exudados biológicos (oral, uretral, rectal y nasal) de pacientes virémicos.
Regresión de la hepatopatía en pacientes VIH que erradican la hepatitis C.
La eliminación del VHC con DAAs modifica la historia natural de la hepatopatía. Sin embargo, un porcentaje significativo de ellos mantienen alteraciones fisiopatológicas características de esta enfermedad persistiendo un riesgo residual de complicaciones hepáticas, particularmente en pacientes coinfectados por VIH/VHC, en los que el envejecimiento del sistema inmune es más acusado. Objetivos: Estudiar el impacto de la erradicación del VHC sobre el organismo y biomarcadores de sangre periférica asociados con la regresión de la hepatopatía. Evaluar marcadores predictivos de evolución clínica.
B) Prevención de la hepatitis C
Cribado de infección activa por el VHC en población de alto riesgo.
Diagnosticar la infección por VHC es el primer paso para la cura y su posterior erradicación. Más del 60% de las personas portadoras del VHC lo desconocen, y esta tasa es mayor en las poblaciones de alto riesgo (adicto a drogas por vía parenteral (ADVP), hombres que tienen sexo con hombres (HSH), etc.). Además, las reinfecciones por VHC entre HSH con prácticas sexuales de alto riesgo es el principal obstáculo en nuestro medio para lograr el objetivo de la OMS de eliminar el VHC para 2030. Objetivos: 1) Cribar la hepatitis C activa en población marginal; 2) Análisis filogenético de las cepas de VHC para comprender los patrones de transmisión del VHC.
Desarrollo un kit de diagnóstico rápido de infección activa por el VHC
La lucha contra la hepatitis C exige nuevas estrategias para el cribado y la eliminación de barreras a la vinculación asistencial. Existe una necesidad imperiosa de detectar la infección "activa" por el VHC por métodos simples y reproducibles, especialmente en poblaciones marginadas que están infradiagnosticadas de hepatitis C. Objetivo: Desarrollo, puesta a punto y validación de un kit de diagnóstico rápido basado en la detección del antígeno del core del VHC en sangre (VHC-Agc; diagnóstico en un solo paso) mediante el uso de anticuerpos monoclonales (AcMs) desarrollados en el laboratorio.
C) Infección VIH
Epidemiologia de eventos clínicos en la infección VIH
El Conjunto de Datos Básicos Mínimos (MBDS) del Ministerio de Sanidad es una base de datos clínicos y administrativos que contiene información registrada en el momento del alta hospitalaria, con una cobertura estimada del 97,7% del total de ingresos hospitalarios en hospitales públicos. El MBDS permite evaluar tendencias epidemiológicas de las altas hospitalarias a lo largo de más de 15 años y en un número elevado de pacientes. Objetivos: Estudiar la evolución de los eventos clínicos (infecciones, cáncer, eventos cardiovasculares, muerte, etc.) en pacientes infectados por VIH, con especial atención en los pacientes coinfectados por VIH/VHC.
Inmunopatogenia de la infección VIH
La infección VIH causa un progresivo deterioro de la inmunidad en pacientes naïve, pero la terapia antirretroviral combinada (cART) suele producir una reconstitución inmune, aunque no suele alcanzar la normalidad. La infección VIH provoca un envejecimiento prematuro, a pesar del cART, que se ha relacionado con un mayor riesgo de desarrollar eventos no relacionados con SIDA. Múltiples factores, como las coinfecciones virales, pueden acelerar o intensificar este proceso. Objetivos: Estudiar biomarcadores asociados con: i) control virológico e inmunológico de la infección VIH en no progresores a largo plazo (LTNPs) y controladores de elite. ii) respuesta virológica y reconstitución inmune en pacientes en cART. iii) senescencia y aparición de eventos no-SIDA en pacientes con cART, así como el impacto de coinfecciones virales (hepatitis (B, C, D, y E), herpesvirus (CMV, EBV, HHV-8,….), etc.). iv) respuesta inmune Ag-específica frente a la infección natural y frente a la vacunación de COVID-19.
